All development starts with requirements. All requirements must trace to features implemented in SW and must be independently verified in functional test. All changes and new features must start with a formal and reviewed Change Request. Changes to requirements, SW, and functional test must trace to a reviewed Change Request and be limited to the scope of the Change Request. All changes driven by the Change Request must be peer reviewed.
