Management abuses of 'fake it till you make it'... No clear vision from their side brings a lot of interesting initiatives to fail. They don't trust and value all the very skillful people they have/had. They have often burnt out their employees and fired them suddenly - smart ones have left before it could happen...
Understandable when you know that management changes every 6 months...
