PTO, high turnover, scattered, etc.

This organization is the definition of dysfunctional. They seem to have no idea how to operate in the 21st century both from a technical perspective and from a benefits perspective. Their offices occupy a dark grungy moldy building that should have been torn down 20 years. It's giving asylum vibes with no windows to speak of. They can't be bothered to spend any Capitol to even provide office furniture that doesn't look like it was acquired 3rd hand. Leadership (notably all family members or friends of family) spends all their time arguing personal agendas instead of leading. Meetings never actually lead to decisions, everyone walks away and just does what they wanted to in the first place regardless of what was said in the meeting. There is no training program for new employees. They fire at least one person per week seemingly out of the blue. There is no retirement benefits, PTO is 5 days starting. Yes you read that right. It will take you 10 years to get to 10 days.
