The bureaucracy gets worse and worse, actually hampering the people on the front lines from doing their jobs. For physicians, part of their compensation starting next year will be provided as an incentive, the bulk of which is for seeing greater numbers of patients. Doctors hate this -- we are taking care of peoples' health. This takes time. Some doctors and some clinics are double and triple booked. They may get a big bonus but the patients will suffer. The organization runs on people paying back the National Health Service Corps, who leave after their 4 years of payback. So it doesn't really matter to management. Little is done to retain providers over the long haul, which is terrible for patients. Many bad-for-morale management decisions that are short sighted (many snow days last winter so management took away 3 holidays....very depressing especially for lower level workers like medical assistants who work really hard for very low pay).

Communication is poor between employees and management. Turnover is quite high, most providers stay with the organization out of a contractual obligation to the National Health Service Corps. The management seems to rely on this system for its staffing, rather than focusing on retention. So, most providers leave after their required years are up. Clinics are understaffed and underresourced simply due to lack of communication/organization. Feedback mechanisms are either not in place, or operate unprofessionally. My training and previous work experience focused on indigent care, which I loved, but after this experience I was so dejected and burned out I decided to move into the private sector.
