Depixen interview questions

I applied for their İstanbul office, and their HR requested to fill a form in advance without telling their name... The survey contained non-inclusive questions like how far you are away from the office...1 hour is our limit..and the options are weird, like I'm in this or that location and less than hour. They are also asking if you're smoking or not with details, like a social smoker or regular. I found the HR like non-human since they refused to tell their name. I do not looking forward to work a place like this.
