Phone interview first with recruiter approximately 30 minutes long. Advised at end of interview that recruiter would next schedule a 1 hour video interview with 2 underwriting supervisors. That interview was scheduled for a few days later. I was then offered the position the next day.

For example: A time you went above and beyond to help a customer
It wasn't too bad they mostly ask you about the core values of the company. Make sure to study those and you will be fine. They are very nice. I got a better opportunity so ended up declining the offer.
Automated phone screen after application was submitted. Very difficult to convey your personality when you are basically waiting for the beep to begin recording. Asked for desired salary and I believe that weeds the most out from getting an interview regardless being negotiable.
