Autocratic management culture. Senior managers put fear in everyone by threatening write ups and termination on a daily basis, and that's without properly training their employees first. Managers do not cover areas when they delegate an overflow of work. I have worked in the Transportation industry for 25 years and have never experienced a more unorganized and poorly trained group of managers.
