Poor management with inconsistent rules; too many things promised but there's no realizations to it, and only bluffed when requested; late payment (though fortunately, they've been on time for the past few months); top management tends to micromanage projects without understanding the full context; there's no proper training if there's a demand to use new things; team structure often lacking (e.g. no Tester while there's a need for it); management tends to blame employees for "doing nothing" while there's no clear work assignment at the moment; career advancement opportunities are unclear; quite high turnover rate.
